Class Plastics (Aust) Pty Ltd manufactures plastic bottles, drums and other plastic storage containers by using a plastic blow moulding machine.

The moulding machine is a large piece of equipment that has an interlock system on its doors that prevents operation when the doors are open. However, on the day of the incident, this system was disabled because tape was placed on the interlock sensors.
